Aircrew Fly Through Radiation for a Living, Should Airlines Track Every Pilot's Lifetime Dose?

A JAMA Internal Medicine analysis of more than 12 million U.S. death certificates from 2020 to 2024 ranked flight attendants first and pilots second among more than 500 occupations for the share of deaths attributed to radiation-linked cancers. Cabin crew had about 50 percent higher odds than other workers. Pilots had about 36 percent higher odds. Radiation-associated cancers accounted for roughly 6.9 percent of flight-attendant deaths and 6.7 percent of pilot deaths, against about 5 percent in the wider working population. Breast, central-nervous-system, prostate and melanoma deaths were elevated in both groups. Pilots also showed higher leukemia mortality. Aircraft mechanics, who work the same machines on the ground, did not. Nuclear technologists ranked twelfth. The authors estimate aircrew receive about 3-6 millisieverts a year from cosmic radiation at cruise altitude-an order of magnitude above a frequent passenger. The study does not measure each person’s dose. It is a mortality pattern strong enough to reopen a workplace question airlines have treated as weather.

Cosmic radiation is not a cabin rumour. At 35,000 feet the atmosphere is a thinner shield. Dose rises with altitude, time aloft, latitude and solar activity. Polar and near-polar tracks used on India-North America and Europe-Asia long-haul flying collect more than equatorial hops. A roster that only counts block hours is not a radiation roster. It is a duty-time roster wearing a safety badge.

Europe already treats aircrew as occupationally exposed. EU rules require monitoring when annual dose may exceed 1 millisievert, tighter management toward 6 millisieverts, training and records. Germany logs cockpit and cabin doses in a national register. Aircrew are a small share of monitored workers and a large share of the collective occupational dose. The United States has long relied more on guidance and company models than on a mandatory individual register of the European kind. That gap is why a U.S. death-certificate study lands as a labour issue as well as a medical one.

Asia is uneven. South Korea has validated airline dose-assessment software against thousands of in-flight measurements and cut the crew annual limit to 6 millisieverts. Japan manages exposure under official guidelines, with published means around 2 millisieverts and maxima near 4 on international flying. India and much of Southeast Asia still treat cosmic radiation as an imported European concern rather than a roster input. Pregnant crew are the sharpest test. International guidance is to keep fetal dose low. That only works if the airline knows last month’s route mix before next month’s pairing is locked.

Lifetime dose is the missing product. A pilot who spends twenty years on polar widebody sectors is not the same worker as a short-haul captain on tropical sectors. Without a portable record that follows the licence, no airline, union or regulator can answer a basic question after a diagnosis. The question is how much did this person actually receive? Modelled route doses are better than nothing. They are not a film badge.

The study’s authors say the pattern supports occupational protection, not a grounded fleet. That is the correct scale. Airlines should publish how they estimate dose, how polar flying is spread, how pregnancy is rostered, and whether a crew member can see a running total. Regulators in Delhi, Singapore and Seoul should decide whether aircrew belong in the same monitoring family as other radiation workers. Flying through radiation is part of the job. Flying without a lifetime account of it is a choice.
