India moves to RED this week after an Air India A320neo operating Phuket–Delhi experienced a sudden altitude variation in cruise on 4 August, injuring 17 people. The occurrence has been classified as a serious incident, AAIB is investigating, and DGCA has removed both pilots from roster pending the investigation and a confirmatory laboratory result relating to the pilot-in-command’s mandatory psychoactive-substance screening.
Asia is AMBER: the week included an Air India safety event originating in Thailand, a TCAS resolution advisory involving an ANA Boeing 767 on approach to Tokyo Haneda, and a continuing warning that GPS/ADS-B interference has been reported by crews around Manila. The global score remains AMBER, with serious but contained operational events and newly issued airworthiness actions, not a new large scheduled-airline fatal accident, defining the period.
1. Week in Safety
The defining event of 3–9 August was Air India flight AI2379, an Airbus A320neo registered VT-EXO, operating from Phuket to Delhi. The Ministry of Civil Aviation said the aircraft experienced a momentary altitude variation in flight on 4 August, later stabilised, and landed safely at Delhi. The flight carried 137 passengers, including three infants, and eight crew members.
Seventeen people, 13 passengers and four cabin-crew members, were reported injured after the event, which initial accounts described as a severe turbulence-related occurrence and approximately 300 feet of altitude loss during cruise. The aircraft was grounded for examination, and the flight recorders were secured by investigators.
On 9 August, the Ministry classified the event as a serious incident and confirmed that AAIB is leading the investigation. Both flight crew members underwent the mandatory psychoactive-substance screening required after such an occurrence; the pilot-in-command’s initial result required confirmatory testing, while the final laboratory report remains awaited.
A second Indian occurrence followed on 9 August at Baramati, Maharashtra. A Carver Aviation flying-academy Cessna 172, VT-SEX, veered off the paved surface during a training exercise and sustained damage to its propeller, engine cowling and nose landing gear. The two occupants were unharmed; DGCA’s air-safety team is conducting a preliminary investigation.
2. India Safety Watch
AI2379 is the week’s central safety story because of its combination of injuries, a material in-flight altitude event, a serious-incident classification, and a parallel screening issue that must be resolved strictly through evidence, not speculation. The current known facts establish only that the aircraft stabilised and landed safely, injuries occurred, the crew has been removed from duty pending procedures, and the investigation is active. The eventual findings must distinguish weather, operational handling, aircraft systems, cabin preparedness and any human-performance factors.
The Baramati runway excursion is a separate but relevant signal for India’s training ecosystem. No injury occurred, but the damage to the nose gear, propeller and cowling shows why runway-excursion prevention, touchdown planning, stopping margins and instructor supervision require continuous attention at flight-training organisations—not only at airline hubs.
The week also brought a broader warning on technical-event reporting. Ministry data released on 3 August showed 352 significant technical defects reported in Indian domestic and international flight operations through June 2026, including 334 involving domestic services. The figures are reports of defects, not equivalent to accidents or proof of unsafe flying, but they reinforce the value of disciplined reporting, follow-up maintenance and regulator oversight as the network expands.
3. Asia Safety Watch
Tokyo Haneda produced the region’s most notable collision-avoidance event of the week. On 4 August, an ANA Boeing 767-300 arriving from Shanghai and descending on final approach to runway 34R initiated a go-around after receiving a TCAS resolution advisory involving a Cessna Citation 525. The aircraft landed safely; the event is a reminder that traffic-alert and collision-avoidance systems remain a vital final layer of protection during high-workload terminal operations.
In Southeast Asia, SafeAirspace reported a high volume of crew reports involving GPS interference in the Philippines, particularly around Manila’s Ninoy Aquino International Airport. The reported effect includes GPS and ADS-B dropouts, which can increase navigation and surveillance complexity for crews and air-traffic services. Operators using the Manila area should treat this as a live operational planning issue and ensure crews understand relevant contingency procedures.
The immediate regional lesson is that aviation safety is increasingly affected by risks outside the aircraft itself, signal integrity, terminal-airspace complexity, weather exposure and systems resilience. The Tokyo TCAS event and reported Manila interference are distinct occurrences, but both underline the importance of independent situational-awareness layers when normal navigation or separation assumptions are degraded.
4. Investigation Update
The Air India AI2379 inquiry is now formally an AAIB serious-incident investigation. Investigators have secured the flight data recorder and cockpit voice recorder, while the aircraft has been moved to a hangar for examination. The investigation must establish the factual sequence before attributing cause.
For publication and public discussion, one principle is essential. The initial psychoactive-screening result is not a final finding. The sample has been sent for confirmatory laboratory analysis, and the Ministry has said further action will depend on the completed investigation and test results. Reporting should avoid treating the preliminary screening information as proof of impairment or causation.
The longer-running Air India AI-171 investigation also remains a national watch item. AAIB told the Supreme Court in late July that it expects to submit its final report by October 2026, following international participation, technical reconstruction and overseas forensic examination of certain components.
5. Regulatory Radar
The United States issued a series of airworthiness actions during the week that deserve global operator attention.
The FAA issued an emergency airworthiness directive for certain Airbus A319, A320 and A321 aircraft requiring replacement or modification of software controlling elevator-ailerons. The directive is effective immediately and requires the prescribed work before the affected aircraft fly again, subject to the FAA’s compliance deadline.
The FAA also issued an immediately effective AD for certain Boeing 787-8, 787-9 and 787-10 aircraft, requiring inspection of captain’s and first-officer’s seats for missing or cracked rocker-switch caps and cracked or non-functional switch cover assemblies.
A further FAA directive applicable to Boeing 787-8, -9 and -10 aircraft addresses insufficient protection of right-engine systems from possible cross-engine debris following an uncontained left-engine failure. The unsafe condition could, if uncorrected, create the risk of damage to critical right-engine systems and a potential dual-engine shutdown.
For Indian operators, these notices are not evidence of an immediate fleet-specific issue in India. They are reminders that maintenance and continuing-airworthiness teams must rapidly assess foreign directives against their fleets, configurations, service bulletins and local regulatory requirements.
6. Global Safety Story: When Air Traffic Control Goes Dark
The most important global safety story of the week was the Minneapolis Center outage. On 6 August, an equipment or telecommunications failure knocked out radar and radio communications at a major FAA facility controlling roughly 330,000 square miles of U.S. airspace. The system recovered, but only after ground stops, diversions and widespread traffic restrictions were used to keep aircraft safely separated.
The event is a reminder that aviation safety depends on infrastructure redundancy as much as it does on aircraft reliability and pilot performance. When an ATC centre loses surveillance and communications, the system must rapidly fall back on procedural controls, adjacent-centre coordination, lower traffic volume and conservative decision-making. The fact that flights were grounded and diverted was evidence of the safety system working under failure conditions, not evidence that the event was routine.

AI2379 landed safely, but 17 injuries show how quickly a short-duration event can affect people in the cabin. The safety chain is not limited to pilots and aircraft systems: it includes weather information, dispatch planning, seat-belt compliance, timely cabin-service suspension, cabin-crew communication and medical response after landing.
The serious-incident classification is therefore appropriate for scrutiny, not a verdict on cause. The investigation should identify whether the altitude variation was exclusively turbulence-driven or whether other operational or technical contributors played a role. Until then, the most responsible industry response is to protect evidence, avoid premature conclusions, support those injured and apply lessons across the network.
9. Weather and Airspace Hazards
Monsoon season remains an active operational factor for India and the region. For crews and dispatchers, the practical risks include convective weather, turbulence, unstable approaches, windshear, reduced visibility, runway contamination and diversion pressure. The AI2379 event makes disciplined turbulence risk management especially urgent this week.
In the Philippines, reported GPS/ADS-B disruption around Manila requires heightened navigation awareness and readiness to use appropriate alternative procedures. In conflict-risk planning, the Middle East remains operationally fluid, with reports of potential cancellations and periodic airspace closures amid a continuing regional security situation.
